How you register with us as a new patient

Patients can either collect a registration form from the surgery or print this  registration form bilingual.pdf 

Please also print the new patient questionnaire.doc and give the completed forms to the reception team along with two forms of Identification one showing your current address and a photo ID such as a passport or drivers licence, Identification documents are not required for children

Please ensure that the completed form signed at the bottom of the first page and that you have included your NHS number  which is a 10 digit number and  you can obtain this from your previous surgery if you do not know what it is.

If you take any repeat medications please give the surgery a copy of your repeat order form.

Completed forms can either be posted to either surgery or handed over to the reception team.

Registration forms can also be obtained from the reception in both surgeries. Please allow 48 hours for your registration to be completed before making an appointment. Your medical notes from your previous surgery can take 6-8 weeks to be forwarded on to us from the Health Authority.

Please note we require your NHS number to process your registration,  if you have been registered previously with the NHS you will have a 10 digit number if you don't know this information you can ask your previous GP Surgery for this information so that we can complete your registration.

If you are arriving in the UK and registering for the first time please complete the relevant section on the registration form.
