Repeat Prescriptions

By telephone	We no Longer take prescription requests over the telephone please use one of the methods below.
In Person	You Can give us your repeat order form or complete a form in reception alternatively you can post it through the letter box.
By Post	To Llandaff Surgery 19aHigh Street Llandaff Cardiff CF5 2DY or The Portacabin Village Hall Heol Y Bryn Pentyrch CF15 9PY stating which pharmacy you would like your prescription sent to.
On Line	Using the My Health online Service full details can be found here My Health Online

Via your pharmacy - you can ask them to put the request in on your behalf

Via email from the surgery enquiries.llandaff@wales.nhs.uk or enquiries.pentyrch@wales.nhs.uk

It is a minimum of 48 hours for collecting a prescription, if you prefer to collect from a local pharmacy please let us us know.
