We are currently accepting new patients - so if you are looking for a GP and live within our practice boundary then you are free to register with the practice. You can call into the surgery to collect the registration form or you can now print off this Registration Form
Please ensure that the completed form is signed at the bottom of the first page and that you have included your NHS number which is a Ten Digit number, you can obtain this from your previous surgery if you do not know what it is.
If you take any repeat medications please give the surgery a copy of your repeat order form.
Completed forms must be handed in in either surgery to the reception staff so that we can see your proof of address and photo ID for all adults .
Registration forms can also be obtained from the reception in both surgeries. Please allow 48 hours for your registration to be completed before making an appointment. Your medical notes from your previous surgery can take 6-8 weeks to be forwarded on to us from the Health Authority.
Please note we require your NHS number if you have been registered previously with the NHS so we can process your registration. If you're registered with a GP, you will already have an NHS number. Your NHS number should be on any letter or document you have received from the NHS, such as prescriptions, test results, and appointment letters. Contact your GP practice and ask them to look it up for you.
